It enables communication among team members, maintains continuity and quality of care, serves professionals from a legal perspective, and also allows for further data processing for research purposes (Harkreader and Hogan, 2004, cited by Ram\u0161ak, 2006). <\/p>\n<p>For well-completed and managed documentation, the Nursing and Midwifery Council of Great Britain (2009) issued guidelines for good documentation. Some of these are: <\/p>\n<ul>\n<li><em><strong>handwriting should be legible,<\/strong><\/em><\/li>\n<li><em><strong>all completed documentation should be signed,<\/strong><\/em><\/li>\n<li><em><strong>records should be accurate and written in a way that clarifies their meaning, \u2026<\/strong><\/em><\/li>\n<\/ul>\n<p>An important factor for well-completed documentation is also the appropriate use of terminology. In 2014, the Maribor maternity hospital introduced modern midwifery documentation, which includes midwifery terminology. Thus, terms such as &#8216;nursing care&#8217; and &#8216;patient&#8217; were replaced with &#8216;midwifery care&#8217; and &#8216;birthing woman&#8217; (Franc, 2014).  <\/p>\n<p><strong>PAPER-BASED DOCUMENTATION<\/strong><\/p>\n<p>Despite advanced technology, the midwifery care process in Slovenia largely remains paper-based to this day (Prijatelj, 2005). Among the most common documentation completed and maintained by midwifery professionals are the maternity record book (Novak, 2004), the birth record (NIJZ, 2016), and the clinical pathway (\u0160trumbelj and Lu\u010dovnik, 2014). <\/p>\n<p>The maternity record book was introduced in Slovenia in 1986 within the Perinatal Medicine Section. Two years later, the Rules on the Implementation of Preventive Health Care at the Primary Level (1998) were adopted, which mandate the issuance of a maternity record book at the first preventive examination during pregnancy. This document serves as a common source of information about the pregnant woman, the course of pregnancy, childbirth, and the newborn (Novak, 1994). Pregnant women must carry their maternity record book with documents of additional examinations and keep it carefully (Novak, 2004).   <\/p>\n<p>The birth record is midwifery and medical documentation that ensures standardized and comprehensive care for the birthing woman during labor. It contains the birthing woman&#8217;s personal data, her medical history, information about previous births, details about the onset, course, and outcome of the current birth, and data about the newborn. It also provides a pictorial overview of labor progression using a partogram, which records cervical dilation and the descent of the presenting fetal part (Lavender et al., 2013).  <\/p>\n<p>In the field of obstetrics in Slovenia, two clinical pathway documents have been developed to date: Standard Care for Pregnant Women in Antenatal Care and Normal Birth. Both documents precisely define when a pregnancy examination or birth is led by a midwife and in which cases the midwife is obliged to call a doctor for further treatment (GIZ, 2016).  <\/p>\n<p>The importance of introducing the Normal Birth clinical pathway is demonstrated by a comparative study conducted by the Ljubljana maternity hospital in 2013. Its aim was to determine the differences in birth outcomes between those performed in an independent midwifery unit and a standard labor unit. One of the biggest differences was observed in the acceleration of labor with oxytocin: in the midwifery unit, acceleration was needed in just over 31% of cases, while in the standard labor unit, it was as high as 84%. Vacuum extraction was used only once in the midwifery unit, compared to 16 times in the control group. A noticeable difference was also found in the rate of C-sections; the need for the procedure was 43% higher in the standard labor unit than in the midwifery unit (Prelec, 2013, cited by Prelec, 2014). The introduction of the clinical pathway for managing normal birth also resulted in a 50% reduction in episiotomy rates and one of the lowest percentages of severe perineal injuries in Europe. Consequently, transferring the care of normal birth to midwives also ensures the psychological safety of the birthing woman, as she does not feel like a patient (GIZ, 2016).      <\/p>\n<p><strong>ADVANTAGES OF ELECTRONIC DOCUMENTATION<\/strong><\/p>\n<ul>\n<li><strong>Improved continuity of care;<\/strong> Accurate, relevant, and up-to-date information is available (Ram\u0161ak, 2006). Accessibility also increases the possibility of interdisciplinary communication, for example, between healthcare personnel working at primary and tertiary levels. Often, patients are in pain, confusion, and distress, making them unreliable sources of data. With electronic documentation, all important data are immediately available (van Bemmel and Musen, 1997).   <\/li>\n<li><strong>Increased quality of care;<\/strong> primarily due to fewer opportunities for communication errors. The advantage of electronically managed documentation lies in standardized terminology, data legibility (van Bemmel and Musen, 1997), and a unified data collection system (Ram\u0161ak, 2006). <\/li>\n<li><strong>All data is entered into the computer system only once.<\/strong> This gives the midwife more time to dedicate to the patient, and her work is performed with higher quality (van Bemmel and Musen, 1997).<\/li>\n<li><strong>Improvements in ecological<\/strong> (Kova\u010di\u010d, 2013) <strong>and financial areas<\/strong> (Guojun et al., 2011). Paper consumption, X-ray films, and fuel that patients would use for transportation from doctor to doctor when carrying documentation are significantly reduced. Consequently, this also affects the reduction of carbon dioxide emissions in the air (Turley et al., 2011).  <\/li>\n<li><strong>Enables the storage of a large number of important data.<\/strong> The advantage over paper documentation is reflected in the reduced possibility of data loss (Prijatelj, 2016).<\/li>\n<li><strong>Data analysis is faster and more accurate<\/strong> (Kova\u010di\u010d, 2013). In addition to textual data about the patient, visual materials can also be entered into the computer system. Stored data can then be displayed and processed not only in textual form but also in tables and graphs, which provide easier understanding (Purkart, 2016).  <\/li>\n<\/ul>\n<p><strong>DISADVANTAGES OF ELECTRONIC DOCUMENTATION<\/strong><\/p>\n<ul>\n<li><strong>Protection of personal data;<\/strong> Due to data accessibility, the possibility of unauthorized persons accessing it increases. However, identity theft is also possible from paper documentation, but the risk is lower. It is important that data is adequately and sufficiently secured (Roberts and Bailey, 2012).  <\/li>\n<li><strong>Lack of familiarity with the computer system and its operation;<\/strong> A study conducted by the University of California showed a 25-33% drop in care productivity in the first weeks of electronic documentation implementation. It is necessary for the healthcare institution to provide timely and sufficient training for employees (Palma, 2013). <\/li>\n<li><strong>Possibility of copy \u2013 paste function<\/strong> (<em>copy \u2013 paste<\/em>); This shortcut can be abused, especially in routine midwifery activities, where changes indicating certain irregularities are quickly overlooked. Although such an action can save the midwife some time, it reduces the safety of the patient&#8217;s health and consequently the quality of care itself (Palma, 2013). <\/li>\n<li><strong>Financial and ecological aspects;<\/strong> Costs arise from updating and upgrading the computer system. Each healthcare institution must also allocate some money for computer training for employees. Due to the increased number of computers, energy consumption and discarded computer parts that break down over time would consequently increase (Roberts and Bailey, 2012).  <\/li>\n<li><strong>Loss of information;<\/strong> Computers are often exposed to viruses that can lead to system crashes. In this case, important data that has accumulated over several years can be lost. Many practices opt for maintaining a backup copy that allows data recovery (Roberts and Bailey, 2012).  <\/li>\n<\/ul>\n<p><strong>EXAMPLE OF GOOD PRACTICE<\/strong><\/p>\n<p>At the Clinical Department of Pediatric Surgery and Intensive Care (KOOKIT) of the University Medical Centre Ljubljana, all nursing documentation is carried out electronically (Purkart, 2016). The clinical information system called <strong>Think!Med<\/strong> is considered one of the most modern not only in Slovenia but also in Europe. The system offers possibilities for managing and documenting vital functions, laboratory tests, procedures, and medications (Mencigar, 2016).   <\/p>\n<p>With the help of the protocol provided by ThinkMed, the doctor determines which <strong>vital functions<\/strong> need to be monitored in the patient. All bed units have their own monitor, and all are connected to a central monitoring station. This provides midwives with a continuous overview of the patient&#8217;s condition. All data from the central station can be automatically transferred and saved to an e-form with a click, as shown in the image. The clinical information system then displays the stored data in descriptive, tabular, or graphical form (Mencigar, 2014).    <\/p>\n<p><img decoding=\"async\" class=\"wp-image-3355 alignleft lazyload\" data-src=\"https:\/\/mojababica.si\/noviteta\/wp-content\/uploads\/2025\/07\/dokum.jpg\" alt=\"docum\" width=\"353\" height=\"452\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 353px; --smush-placeholder-aspect-ratio: 353\/452;\"><\/p>\n<p>&nbsp;<\/p>\n<p><em>Figure 1: Entry of vital functions into the clinical information system &#8211; Think!Med (Mencigar, 2014).<\/em><\/p>\n<p>ThinkMed&#8217;s role in the process of performing <strong>laboratory tests<\/strong> is to order the test and read the samples, and then, after the analysis is completed, display the results in numerical, tabular, or graphical form, and can also compare them (Mencigar, 2014).<\/p>\n<p>When documenting performed <strong>procedures<\/strong>, ThinkMed ensures greater traceability and safety. For example, the documented insertion of a catheter is displayed in pictorial form (Mencigar, 2014). <\/p>\n<p>&nbsp;<\/p>\n<p><img decoding=\"async\" class=\"size-full wp-image-3356 alignright lazyload\" data-src=\"https:\/\/mojababica.si\/noviteta\/wp-content\/uploads\/2025\/07\/dokum2.jpg\" alt=\"docum2\" width=\"370\" height=\"286\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 370px; --smush-placeholder-aspect-ratio: 370\/286;\"><\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><em>Figure 2: Overview of inserted foreign bodies (Mencigar, 2014).<\/em><\/p>\n<p>When <strong>prescribing medications<\/strong>, the clinical system provides information on indications and contraindications for the use of individual drugs, warns of their side effects, checks for possible drug allergies, their interaction with food, and much more. Once the doctor confirms all prescribed therapy, it is displayed in tabular or graphical form, as shown in the image, which allows for a better overview (Mencigar, 2014). <\/p>\n<p>In the last few decades, midwifery documentation has undergone remarkable progress. From oral transmission of health information, it has evolved to written documentation in paper form. All this is due to the need for COMPREHENSIVE, PROFESSIONAL, and ACCURATELY MAINTAINED midwifery care. The technological development we are witnessing today once again represents a significant turning point in the field of midwifery documentation.
