Before traveling, it makes sense to discuss possible risks with your doctor or gynecologist. For most women, traveling during pregnancy is safe. As long as the pregnant woman and fetus are healthy, they can safely travel until 36 weeks of pregnancy. The best time to travel is mid-pregnancy—between weeks 14 and 28 (Abteilung für Infektions- und Tropenmedizin der Ludwig Maximilian Universität, 2009; ACOG: The American Congress of Obstetricians and Gynecologists, 2017).

The most common pregnancy problems occur in the first and third trimesters. Pay attention to how you feel—let your well-being guide your activities.

Since only a few medications are completely safe during pregnancy, a thorough risk-benefit assessment regarding travel is advisable. Treating infections can be quite complicated, but no necessary treatment should be withheld due to fear of potential harm/illness to the fetus. Good knowledge of local medical resources (hospitals, care facilities, etc.) is essential before traveling.

Multiple personal protective measures reduce the risk of developing an infection: dietary and water precautions, protection against insect bites and avoiding crowds, avoiding unprotected sex and standing water.

When is travel during pregnancy not recommended?

In case of pregnancy complications, travel is not recommended. Such cases include (ACOG: The American Congress of Obstetricians and Gynecologists, 2017):

– preeclampsia,

– preterm premature rupture of membranes (PPROM, PROM)

– when there is a risk of preterm labor (PTL)

You should consider whether travel during pregnancy makes sense if you have the following complications (American Pregnancy Association, 2017):

– Severe anemia

– Heart disease

– Respiratory diseases

– Recent bleeding

– Current or recent bone fractures

Deep Vein Thrombosis and Travel During Pregnancy

Pregnancy further increases the risk of deep vein thrombosis (DVT). During long trips and air travel, drink sufficient fluids, wear loose clothing, walk and stretch at regular intervals (at least every 2 hours). It is important to promote blood circulation. If you cannot get up during the flight, frequently bend, flex, and extend your ankles. Also avoid wearing tight clothing. Using compression stockings can help prevent blood clots (Butler Tobah, 2017).

If you are traveling by car, stop frequently and take a walk, then gently lift each leg separately (Mann, 2017). Before flying, avoid foods that cause bloating and carbonated drinks. Always wear your seatbelt (ACOG: The American Congress of Obstetricians and Gynecologists, 2017).

Traveling by Car During Pregnancy

Fatigue and dizziness are common during pregnancy, so it is important to drink regularly when traveling by car, eat naturally energy-rich foods (such as fruit and nuts), and take regular breaks at rest stops (at least every 2 hours).
Make sure there is fresh air in the vehicle cabin and use a seatbelt that is fastened so that the upper part of the belt runs between the shoulder and neck and forward between both breasts. The lower part of the belt should run across the pelvis, under the belly and not over the belly.

Traffic accidents are among the most common causes of injury in pregnant women. Avoid long car trips when you are alone; it is advisable to have a companion (NHS – Travelling in pregnancy, 2016).

Flying, Radiation, Diving, High Altitudes, and Pregnancy

If the pregnancy is progressing without complications, most airlines allow pregnant women to fly until week 36 (domestic flights) and week 35 (international flights) of pregnancy. If the fetus’s oxygen supply is not already compromised under normal conditions on the ground, for example due to an underlying disease, flying does not pose a risk of fetal hypoxia. Radiation exposure during a long flight is low compared to average annual exposure, but the risk of thrombosis increases. Altitudes up to 2,500 meters should not pose problems. When traveling to high altitudes, sufficient time should be devoted to acclimatization, and physical exercise should be kept to a minimum. Diving is contraindicated and not recommended during pregnancy (Abteilung für Infektions- und Tropenmedizin der Ludwig Maximilian Universität, 2009).

Traveling by Ship and Pregnancy

Especially on longer trips, make sure there is a doctor or nurse on board. Also make sure that your planned stopovers are places with modern medical centers. Ask your doctor or gynecologist which medications are safe to use during pregnancy in case of seasickness.
Cruise ships pose a risk of norovirus infection. Noroviruses are a group of viruses that can cause diarrhea and vomiting lasting 1-2 days. People easily become infected through contaminated food and drink, or by touching surfaces contaminated with the virus. While on board, wash your hands frequently. If you have both diarrhea and vomiting, seek medical attention (ACOG: The American Congress of Obstetricians and Gynecologists, 2017).

Travel to Exotic Destinations and Vaccines

People traveling to exotic destinations are recommended to get vaccinated, but some live vaccines may be contraindicated in pregnant women. Exceptionally, after the first trimester, vaccination against yellow fever may be given. Dead, inactivated, or polysaccharide vaccines can be administered after the first trimester only after a thorough risk/benefit assessment. Due to the potential life-threatening danger to mother and fetus after malaria infection, travel to regions where malaria is common is not recommended. If the risk of malaria infection is high, chemoprophylaxis with mefloquine is indicated (Abteilung für Infektions- und Tropenmedizin der Ludwig Maximilian Universität, 2009). In general, the advice is not to travel to exotic destinations, places with extreme weather conditions, and politically unstable countries with high crime rates or where war is raging during pregnancy.

When should you seek medical attention while traveling during pregnancy?

– if vaginal bleeding occurs
– if you experience chest pain, abdominal pain, or contractions
– in case of rupture of membranes (if “water breaks”)
– with signs and symptoms of preeclampsia (headache that won’t go away, flashing before the eyes, pressure in the pit of the stomach or other vision changes, swelling of the face or hands)
– in case of severe diarrhea or vomiting
– with signs of deep vein thrombosis (e.g., firm swelling of the affected limb, pain, limb is dark bluish in color, skin becomes warm, subcutaneous veins swell, etc.)

Tips for Easier Travel During Pregnancy:

The American Pregnancy Association recommends that in foreign countries you only consume bottled beverages (water, juices, soft drinks), make sure milk is pasteurized, and avoid fresh fruit and vegetables unless cooked or peeled. It is important that meat and fish are properly cooked; if you are not sure, avoid them.

– Before departure, have a checkup and consult with your doctor or gynecologist.

– If possible, have your doctor’s/gynecologist’s phone number so you can call them in case of problems.

– It is important to know your EDD—estimated due date.

– Don’t forget to take prescribed medications and products you regularly use (e.g., hemorrhoid ointment, first aid kit, prenatal vitamins, etc.).

– Check travel times.

– Take out appropriate health insurance for abroad and don’t forget to arrange for “cancellation coverage”—refund in case of trip cancellation.

– Find out about the locations of hospitals and health facilities in the places you are traveling to.

– If you are traveling far, have copies of your prenatal documents with information about your pregnancy with you.

– Enjoy your trip!

Photo1, Photo2

Source1, Source2, Source3, Source4, Source5

Is traveling during pregnancy safe?
